The exact question, where to find it, and which box means yes.
The question
Ohio law sets the wording of a school district income tax question. For an earned income tax like this one, the ballot reads close to this, with the blanks filled by the certified language:
"Shall an annual income tax of 1.00% on the earned income of individuals residing in the school district be imposed by the Wadsworth City School District, for [the number of years, or a continuing period of time], beginning [the date it would first take effect], for the purpose of [the stated purpose]?"
The three blanks come from the certified ballot language filed with the Medina County Board of Elections. The committee does not have it yet. The day it does, this page quotes it word for word. We do not guess at the years, the start date, or the purpose before then.

Fill in this box to vote yes on the 1.00% earned income tax.
AGAINST THE TAX
This is a no vote
Leaving both boxes empty counts as no vote on the question at all.
Where it is on your ballot
The levy is a school district question. Look for it after the candidate races, in the questions and issues section. The issue number is assigned by the Board of Elections and will appear here once the ballot is certified.
